Fees
. . .
My fee for a fifty-minute Individual or Family counseling
session is $75. A typical therapy session is based on
a fifty-minute clinical hour rather than a sixty-minute
clock hour.
-
A discounted pre-pay "multiple sessions" package
may be offered after the first session if you do not
have insurance.
- Fees
for Public Speaking Engagements or Workshops are variable.
Please contact me for further information.
- Reports,
court appearances etc. may be provided at
additional cost. These costs are normally
not covered by insurance.
Payment
. . .
Unless your insurance has been PRE-APPROVED,
payment is due at each session at the time
services are rendered. I accept checks and
cash.
Insurance
. . .
I am an in-network provider for Magellan
and ComPsych insurance plans
and EAP's (employee assistance programs) associated
with them. I also accept the NMC Priority
Health plan and Health Plan
of Michigan and Wells Fargo
as an out-of-network provider. Fees are accepted
in accordance with these insurance plans and
must be pre-approved.
Some insurance plans do not cover my services;
including Blue Cross Blue Shield
and many Priority Health plans.
Except
for insurance(s) specifically indicated, I am
an out-of-network provider. I can accept the
out-of-network reimbursement for many insurance
plans. If you expect to have your insurance
cover my services, make sure to have your insurance
information available and check with me to confirm
coverage prior to setting up your first appointment.
Otherwise; payment will have to be made by you
at the time of service. Please check your coverage
carefully by asking the following questions:
- Do
I have mental health benefits?
- What
is my deductible and has it been met?
- How
many sessions per calendar year does my
plan cover?
- How
much does my plan cover for an out-of-network
provider?
- What
is the coverage amount per therapy session?
- Is
approval required from my primary care physician?
Services
may be covered in full or in part by other health
insurance or employee benefit plans.
Upon request,
I will provide a completed document with the
necessary diagnostic and service codes to your
insurance company for completed sessions. Your
insurance company will make a determination
as to any reimbursemet you may be entitled to
receive in accordance with your plan.
Cancellation
Policy . . .
Please
call me a minimum of 24 hours in advance if
you need to reschedule or cancel your appointment
in order to avoid being charged for a missed
session. If you do not show up for your scheduled
appointment, and you have not notified me
at least 24 hours in advance, you will be
required to pay the full cost of the session
as booked.
